Prior

  • Fill out your patient portal prior to your visit. Please contact our staff to get an email sent to you regarding sign in if you do not know your username and/or password

  • Email us a picture of the front and back of your current vision and medical insurance cards to era@islandoptometry.com

  • If you are unable to do the patient portal, please bring in a list of your current medications to your appointment

  • If you are ill or have a fever, coughing, shortness of breath or other COVID-19 related symptoms, have had known exposure to COVID-19 or come into contact with an individual who has COVID-19 in the past 14 days, please call and reschedule your appointment 

  • If you feel sick, or have a cough, we ask you to wear a mask during your visit

  • If you are immune compromised or would just prefer those working closely with you to wear a mask, please let our front desk know and we would be happy to accommodate

Arrival & During

  • Please arrive on time for your appointment, we will need to reschedule your appointment if you are 10 minutes or more late 

  • Upon arrival at the office you may enter and check in with the front and wait in the waiting room for your appointment.

  • If you would rather wait in your car, please call or text us from your vehicle at 360 293-2127

After

  • Access your records from your patient portal, please contact our staff if you need help accessing the patient portal
